Ministry Sales, Event, Offering, and Zoom Request Form

Ministry officers should complete this form at a minimum of 2 weeks prior to selling any items to allow time for creation of the form(s), publishing to the app/website, and training of ministry person(s) on using it. Officers have the option of having sales forms for internal use only so that cash/check/credit card can be received at the computer tables only or the sales forms can be made available to also be published on the website and church app as well. Options for the sales forms will be altered at the discretion of the Pastor. Please fill out this form and click submit.

The Media Team exists to help Orchard Knob Baptist Church ministries communicate effectively and consistently. This form is designed to assist ministries with form creation (event registrations, sales, etc.), Zoom events, social media/website requests, money collection, or special collection (offering) funds in Breeze or Givelify.

Submitting a request through this form assumes you are planning ahead and providing as much notice as possible. We require a minimum of 2 weeks’ notice. This helps ensure we have adequate time to prioritize all requests.
