Ministry Sales, Event, Offering, and Zoom Request Form

Ministry officers should complete this form at a minimum of 2 weeks prior to selling any items to allow time for creation of the form(s), publishing to the app/website, and training of ministry person(s) on using it. Officers have the option of having sales forms for internal use only so that cash/check/credit card can be received at the computer tables only or the sales forms can be made available to also be published on the website and church app as well. Options for the sales forms will be altered at the discretion of the Pastor. Please fill out this form and click submit.

For Sales Items:
    When the item(s) will be back, who they can be picked up from, who should be contacted, price, or other items that I can put in the notes or that needs to be put on the customer's receipts or registration forms.

For Events/ Website/ Social Media:
